High School Sports Physicals

Beginning May 1st, 2025, Community Hospital of Anaconda, in partnership with our pediatric care providers, is accepting Southwest Montana students for annual combined well child exams and sports physicals.

• Student athletes can have their sports physical completed during their annual well-child exam at no additional cost. We encourage parents/guardians to schedule a well-child exam as they provide the best opportunity to ask questions about child development, in addition to a comprehensive review of topics like social development and more.
• Student athletes who have had a well-child exam performed by a non-CHA provider within the past year will be charged a $25 sports physical fee at time of visit. Financial assistance is available.
• $25 fee will be waived if a well-child exam has been administered by a CHA provider within the last year.
• Students must be accompanied by a parent/guardian.

Sports physicals must be completed between
May 1st – September 30th; scheduling early is advised.
