CHA Diabetes Education


Community Hospital of Anaconda Diabetes Education Self Management program has been recognized by the American Diabetes Association as meeting the national standards for diabetes education. This program is coordinated by a registered nurse and registered dietitian, both Certified Diabetes Educators, each with over 20 years experience in diabetes education. This program works closely with medical providers to assure the best in diabetes care for those we serve.

The following content areas are to be included based on the patient assessment:


Describing the disease process and treatment options

Incorporation of nutritional management into lifestyle

Incorporation of physical activity into lifestyle

Using medication safely and for maximum therapeutic effectiveness

Monitoring blood glucose and other parameters and interpreting and using the results for self-management decision making

Preventing, detecting and treating acute complications

Preventing detecting, and treating chronic complication

Developing personal strategies to address psychosocial issues and concerns

Developing personal strategies to promote heath and behavior change
